Community Aide -Supported Living Services
Under the direction of the Program CEO/Director and in coordination with contracting government agencies, local Regional Centers, and interdisciplinary team members, the Community Aide Manager is responsible for overseeing the daily operations of the Supported Living Services (SLS) program. This position provides leadership, supervision, and administrative oversight to ensure the delivery of high-quality, person-centered services that promote consumer independence, self-determination, health, safety, and community inclusion.
The Community Aide Manager supervises Community Specialists and Community Aides, monitors program performance, ensures compliance with California Department of Developmental Services (DDS) regulations, Title 17 of the California Code of Regulations, the Lanterman Developmental Disabilities Services Act, Regional Center requirements, and Ability Aides, LLC policies. This position works collaboratively with consumers, families, Regional Center Service Coordinators, community partners, and agency staff to ensure exceptional service delivery and positive consumer outcomes.
General Responsibilities
1. Supervise, mentor, train, and evaluate SLS Community Specialists and Community Aides to ensure consistent, high-quality, person-centered service delivery.
2. Oversee the daily operations of the Supported Living Services program, ensuring services are delivered according to each consumer's Individual Program Plan (IPP) and Individual Service Design (ISD).
3. Ensure compliance with California Department of Developmental Services (DDS), Title 17 regulations, the Lanterman Developmental Disabilities Services Act, HIPAA, Regional Center contracts, and Ability Aides, LLC policies and procedures.
4. Monitor program quality through regular quality assurance reviews, field observations, documentation audits, and corrective action planning.
5. Review consumer documentation, including daily service notes, progress reports, incident reports, behavioral documentation, medication documentation (when applicable), and other required records for accuracy, completeness, and regulatory compliance.
6. Monitor consumer health, safety, rights, and overall well-being while ensuring staff implement appropriate supports and follow all agency policies.
7. Participate in consumer admissions, assessments, service planning, transitions, and Individual Program Plan (IPP) meetings as needed.
8. Develop employee work schedules, assign caseloads, monitor productivity, approve time records, and ensure appropriate staffing to meet consumer needs.
9. Recruit, interview, onboard, train, coach, and retain qualified staff while promoting a positive, professional, and inclusive work environment.
10. Investigate consumer concerns, employee performance issues, incidents, and complaints, implementing corrective action plans when necessary.
11. Coordinate with Regional Center Service Coordinators, healthcare providers, families, conservators, and community partners to ensure continuity of services and effective communication.
12. Monitor program budgets, resource utilization, and operational expenses while maintaining fiscal responsibility.
13. Prepare and review reports, quality assurance data, performance metrics, and other administrative documentation to support agency operations and continuous quality improvement.
14. Promote consumer rights, informed choice, dignity, privacy, self-determination, and community integration while ensuring compliance with all applicable laws and regulations.
15. Maintain office operations, records, scheduling systems, and administrative procedures necessary to support efficient program operations.
16. Respond appropriately to emergencies, critical incidents, and mandated reporting requirements in accordance with agency policies and applicable regulations.
17. Perform other duties as assigned to support the mission and goals of Ability Aides, LLC.
Physical Requirements
Frequently stand, walk, bend, reach, and climb stairs while visiting consumer homes and community locations.
Lift and carry up to 25 pounds as needed.
Operate a motor vehicle safely while traveling between service locations.
Work in office, home, and community-based environments.

Job Qualifications
Community Specialist
Under the direction of the Program CEO/Director and in coordination with contracting government agencies, local Regional Centers, and interdisciplinary team members, the Supported Living Services (SLS) Community Specialist provides leadership, oversight, and quality assurance for the delivery of Supported Living Services. The Community Specialist ensures that services are implemented in accordance with each consumer's Individual Program Plan (IPP), Individual Service Design (ISD), and all applicable California Department of Developmental Services (DDS), Title 17, and Regional Center requirements.
The SLS Community Specialist provides direct supervision, coaching, and support to SLS Community Aides, conducts consumer assessments, monitors service quality, and promotes person-centered practices that maximize consumer independence, health, safety, community inclusion, and quality of life.
General Responsibilities
1. Conduct functional and person-centered assessments for new and existing consumers to identify strengths, support needs, preferences, and areas for skill development.
2. Develop, implement, and monitor individualized service recommendations, goals, and objectives that support the consumer's IPP and Individual Service Design (ISD).
3. Supervise, mentor, and provide ongoing coaching to SLS Community Aides to ensure high-quality, person-centered service delivery.
4. Conduct routine quality assurance visits, field observations, and random quality control audits to ensure services are delivered in accordance with Ability Aides, LLC policies, Regional Center requirements, and consumer goals.
5. Review daily service notes, progress reports, behavioral documentation, incident reports, and other required records for accuracy, completeness, and compliance.
6. Complete quarterly consumer progress evaluations and recommend revisions to service plans when appropriate.
7. Monitor consumer health, safety, behavioral supports, and overall well-being, ensuring concerns are promptly communicated to supervisors, Regional Center Service Coordinators, and other members of the interdisciplinary team.
8. Participate in consumer intake, assessments, transition planning, and service coordination activities.
9. Attend and represent Ability Aides, LLC at Individual Program Plan (IPP) meetings, interdisciplinary team meetings, case conferences, and Regional Center meetings.
10. Serve as the primary liaison between Ability Aides, LLC, Regional Center Service Coordinators, families, conservators, healthcare providers, and community partners.
11. Provide training and ongoing support to staff regarding person-centered practices, consumer rights, documentation requirements, health and safety procedures, behavior support strategies, mandated reporting, and agency policies.
12.Identify service concerns, barriers to consumer success, and opportunities for program improvement while implementing corrective actions as needed.
13. Ensure compliance with the California Department of Developmental Services (DDS), Title 17 of the California Code of Regulations, the Lanterman Developmental Disabilities Services Act, HIPAA, and all Ability Aides, LLC policies and procedures.
14. Maintain confidentiality of all consumer, employee, and agency information.
15. Perform other related duties as assigned in support of the Supported Living Services program.

Job Qualifications
Supported Living Services (SLS) Community Aide (Direct Support Professional)
Under the direction of the Program CEO/Director and in coordination with contracting government agencies, local Regional Centers, and interdisciplinary team members, the Supported Living Services (SLS) Community Aide is responsible for implementing services identified in each consumer's Individual Program Plan (IPP) and Individual Service Design (ISD). This position provides person-centered supports that promote independence, self-determination, community inclusion, health, safety, and quality of life for adults with developmental disabilities living in their own homes.
SLS Community Aides are expected to uphold the highest standards of care, professionalism, and advocacy while supporting each individual's right to choice, dignity, independence, and meaningful participation in their community. Staff assist consumers in developing the skills necessary to achieve and maintain the highest level of independent living possible while balancing independence with health and safety.
General Responsibilities
1. Provide individualized supports that promote consumer health, safety, dignity, choice, and independence in accordance with each consumer's IPP and Individual Service Design (ISD).
2. Teach, model, and reinforce independent living skills, including personal hygiene, meal planning and preparation, housekeeping, laundry, household organization, shopping, money management, communication, problem-solving, self-advocacy, and community safety.
3. Provide assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), including personal hygiene, dressing, meal preparation, housekeeping, and household management, based on each consumer's assessed needs while encouraging maximum independence.
4. Support consumers in accessing community resources, recreational activities, educational opportunities, employment services, volunteer activities, medical appointments, and other community-based programs identified in the IPP.
5. Assist consumers in developing financial management skills, including budgeting, banking, shopping, comparison pricing, and responsible money management while maintaining compliance with Regional Center policies.
6. Identify barriers that interfere with the development of independent living skills and implement strategies that encourage growth, confidence, and self-sufficiency.
7. Encourage consumer participation in setting personal goals, making informed choices, and exercising their rights and responsibilities.
8. Safely transport consumers to and from approved community activities, medical appointments, educational programs, employment, and other authorized destinations using agency or personal vehicles, as assigned.
9. Provide medication reminders, observe for changes in health or behavior, document observations, and promptly report concerns to supervisors. Staff shall not administer medications unless specifically authorized, trained, and permitted under agency policy and applicable regulations.
10. Maintain accurate, objective, and timely documentation, including daily service notes, incident reports, behavioral observations, mileage logs, and other required records in accordance with Ability Aides, LLC policies and Regional Center requirements.
11. Communicate effectively with consumers, family members, Regional Center Service Coordinators, healthcare providers, and other members of the interdisciplinary team while maintaining professional boundaries and confidentiality.
12. Attend and actively participate in Individual Program Plan (IPP) meetings, staff meetings, required in-service trainings, and continuing education opportunities.
13. Protect and promote consumer rights, privacy, dignity, informed choice, and self-determination in accordance with Title 17 of the California Code of Regulations, the Lanterman Developmental Disabilities Services Act, HIPAA, and Ability Aides, LLC policies.
14. Respond appropriately to emergencies, accidents, behavioral crises, and suspected abuse or neglect by following agency procedures, mandated reporting requirements, and emergency protocols.
15. Perform other related duties as assigned to support consumers in achieving their individualized goals and maintaining successful independent community living.

Lift and carry up to 50 pounds as necessary.
Assist consumers with mobility and transfers when required.
Operate a motor vehicle safely while transporting consumers.
